Wesley Manor offers physical, occupational, and speech therapy to help residents regain strength, improve mobility, restore communication skills, and increase independence after surgery, illness, injury, or hospitalization. Every therapy program is personalized to each individual’s goals, abilities, and recovery plan.

Our rehabilitation team works with individuals recovering from joint replacement surgery, fractures, strokes, cardiac procedures, neurological conditions, illness, and general hospitalization. Therapy plans are customized to help each person safely regain strength, confidence, and independence.

Residents are welcome to continue seeing their preferred physicians and specialists whenever possible. Wesley Manor also coordinates care with healthcare providers to help ensure residents receive personalized, comprehensive care throughout their stay.

Every resident receives an individualized care plan created by an interdisciplinary team that includes physicians, nurses, therapists, caregivers, social workers, residents, and family members when appropriate. Care plans are based on each person’s medical needs, rehabilitation goals, preferences, and overall well-being, and are reviewed regularly as needs change.

Our interdisciplinary team coordinates closely with physicians, therapists, nurses, residents, and families to provide comprehensive, personalized care. Wesley Manor works closely with hospitals, physicians, case managers, and discharge planners to coordinate a smooth transition following surgery, illness, or hospitalization. Our admissions and clinical teams help ensure your care begins promptly so you can focus on recovery in a supportive environment.

Wesley Manor’s Health Center is Medicare-certified, allowing eligible residents and patients to receive covered Skilled Nursing and rehabilitation services in accordance with Medicare guidelines. Our admissions team can help explain your benefits and answer questions about coverage.

Wesley Manor provides scheduled transportation to many off-campus medical appointments, making it easier for residents to access healthcare services without the stress of driving or arranging transportation.

Our goal is always to help individuals return to the highest level of independence possible. Depending on your progress, you may return home, transition back to Independent or Assisted Living at Wesley Manor, or continue receiving additional care if needed. Before discharge, our team works closely with you and your family to develop a personalized transition plan that supports a safe and successful recovery.
