We understand the challenges of caring for a loved one with dementia, which is why we offer caregiver support groups at all of our campuses. These groups provide a compassionate space to share experiences, gather resources, and connect with others who understand. Our Wesley Manor’s Memory Care Support Group meets the 4th Tuesday of every month—you do not have to have a loved one living within our community to attend! Memory Care may be the right choice when memory loss begins affecting a person’s safety, independence, or daily routine. While Assisted Living provides support with daily activities, Memory Care is specifically designed for individuals living with Alzheimer’s disease and other forms of dementia. The Journey cares for individuals living with Alzheimer’s disease, vascular dementia, Lewy body dementia, frontotemporal dementia, mixed dementia, and other forms of cognitive impairment. Because every person’s journey is different, care is individualized based on each resident’s diagnosis, abilities, preferences, and stage of dementia.

The Journey provides a secure, thoughtfully designed environment that allows residents to move about safely while reducing the risks associated with wandering or disorientation. Every aspect of the neighborhood is designed to create a comforting, familiar atmosphere that encourages independence while giving families added peace of mind.

Every resident receives an individualized care plan developed in partnership with family members, physicians, and our interdisciplinary care team. We take time to learn about each person’s background, career, hobbies, preferences, daily routines, and healthcare needs so care feels familiar, meaningful, and personalized.

Care plans are reviewed regularly and adjusted as needs change.

Residents participate in daily activities designed to support cognitive function, emotional well-being, physical health, and social connection. Programs may include music, gardening, sensory experiences, art, reminiscence therapy, exercise, games, baking, and other meaningful activities that encourage purpose and joy.
